Regarding your query, there is no such option to customize the pathways given by the breadcrumb in the theme itself. Breadcrumb by default shows the complete path of the individual page starting from the home.
However, if you still want to make the customization in your site, you can refer to our Hire our Customizer service.

Adding the custom made icons requires the changes in the core theme files as the feature is not given even by the pro version of the theme.

To add the form in the contact page, you can use the plugins as Contact Form 7 , design the form with the plugin and configure the form to the specific page by pasting the short code that you will receive with every new form.

It is due to the conflict with the plugin Beaver Builder lite. So, once deactivate the plugin and verify the issue. It should work.
